Sam Otis Hornback

Sam Hornback, age 76, and a resident of Forest, died Thursday, April 10, 2025, following an extended illness.

Graveside services will be held at 2:30pm, Monday, April 14, 2025, at the Oak Grove Church of God Cemetery in the Polkville community. Bro. Gary Morris will officiate under the direction of OTT & LEE Funeral Home of Forest. Visitation will be held from 1pm till 2pm Monday at the funeral home.

Sam was born in Smith County to his parents Paul Lee Hornback and Audelle Nichols Hornback on September 7, 1948. He became a carpenter and worked in the construction field. He will be greatly missed by all who knew and loved him.

He was preceded in death by his parents, and his wife, Linda Diane Hornback.

Survivors include his children, Anita Sistrunk of Richland, Lisa Michelle Richardson (David) of Forest, and Danielle Hornback of Morton. He is also survived by six grandchildren and four great-grandchildren.
